Patient Bill of Rights As a patient receiving services from Hanford Home Health, you are entitled to the following rights.

  • You have the right to quality care—without discrimination.

    Receive the services outlined in your plan of care regardless of race, color, creed, sex, age, national origin, disability, beliefs, religion, or sexual orientation.

  • You have the right to be cared for by qualified professionals.

    All care will be delivered by appropriately trained and credentialed staff, contractors, or volunteers, with coordination across your care team.

  • You have the right to participate in your care plan.

    You’ll be involved in developing and updating your care, ensuring it reflects your needs and preferences.

  • You have the right to choose your care provider.

    Access a list of licensed home health agencies and select a provider that fits your coverage and care requirements.

  • You have the right to know what services are offered.

    You will be informed of all services Hanford Home Health offers, and which of those you are receiving.

  • You have the right to refuse specific services.

    You can decline any service at any time, even if it’s included in your care plan.

  • You have the right to know who oversees your care.

    We’ll provide the name and contact information of the person responsible for supervising your care.

  • You have the right to respect and dignity.

    You will always be treated with courtesy, respect, and consideration for your privacy.

  • You have the right to be free from abuse and neglect.

    You are protected from all forms of verbal, mental, sexual, or physical abuse, including neglect, exploitation, and discrimination.

  • You have the right to respect for your personal belongings.

    Your personal property will be handled with care and respect during all interactions.

  • You have the right to confidentiality.

    All personal and healthcare information, including records, will be kept private and secure.

  • You have the right to understand your financial responsibility.

    We will inform you of any charges for services and what may be covered by insurance, public programs, or other sources.

  • You have the right to an itemized bill.

    Upon request, you can receive a detailed billing statement listing each service and charge. (Note: Managed care plans may differ.)

  • You have the right to be informed about advance directives.

    We’ll help you understand advance directives, POLST forms, and how they relate to the care we provide.

  • You have the right to understand discharge policies.

    We will explain the circumstances under which Hanford Home Health may discontinue services.

  • You have the right to know about backup care options.

    If your scheduled caregiver is unavailable, we’ll inform you of the procedures we follow to ensure continued care.

  • You have the right to file a complaint—without fear.

    You and your family can submit concerns about care or services and have them addressed without retaliation.

  • You have the right to contact the Department of Health.

  • You have the right to report suspected abuse.
